Motor Vehicle Accident Reports and Police Incident Reports

Police Accident Reports:

Accident reports are only available to parties involved in the accident including the owner of a vehicle involved, operator or passenger of a vehicle involved, a pedestrian hit by a vehicle involved, or an owner of any property damaged as a result of the accident. Attorneys representing individuals involved in civil litigation can obtain motor vehicle records at the NH Department of Safety, 10 Hazen Drive, Concord, NH, by filling out a standard DSMV 505 form, pursuant to NH RSA 260:14, V(a)(2).

A written request must be made either in person or through the mail. In order to identify the report requested the following should be included in the request:

  • Case number (if available)
  • Parties involved
  • Date and time of accident
  • Location of accident

There is a processing fee of $15.00 for each full report requested, which must be prepaid and is non-refundable. Please make all checks payable to “Town of Plaistow”. The full report includes diagrams if any, and a brief narrative by the officer. If photos are included in the report and you wish to receive reprints there is an additional charge of $10.00 for reproduction of the photos.

At no charge, you can receive a copy of the face sheet which includes each of the driver’s insurance information.

Please note that we do not fax accident reports and the administration staff will not discuss and/or provide any information contained in the reports over the telephone.

Police Incident Reports:

Any requests for an incident report must be put in writing to the Chief of Police. No reports will be released without the Chief’s authorization. In order to identify the report requested the following should be included in the request:

  • Case number (if available)
  • Parties involved
  • Reason for request
  • Date and time of incident
  • Any information regarding the incident

There is a processing fee of $15.00 for each report requested, which must be paid at the time of the request. Please make all checks payable to “Town of Plaistow”.
