ACCIDENT or INCIDENT REPORT Request

As you enter the main lobby of the Plaistow Police Department, the Records Office window is located to your right. This is where you may request reports or pay fines/fees.

To request a Dispatch Log, Accident, or Incident Report:

  • All requests require the following form to be completed in full, as well as a picture ID when dropping off the request and picking up the report. Click on the form located at the bottom of this page.
  • Forward the completed request form to lchampeno@pd.plaistow.com or drop it off in person at the Plaistow Police Station, 27 Elm Street, Plaistow, NH at the Records window.
  • Reports may not be readily available and could take up to 10 days to be processed.  Open/active case requests will be referred to Prosecution.
  • Reports are only available to parties directly involved in the accident or incident (i.e. vehicle/damaged property owners, drivers, passengers, etc.)
  • We do not email or fax reports.  Reports must be picked up in person. Only the person requesting the report may pick up the report with a picture ID.
  • There is a fee of $1.00/page fee for reports of 5 pages or longer, which must be paid when you receive the report.
  • We accept the following for payment:  credit/debit cards (there is a $2.50 processing fee, in addition to the page fee) and personal checks/money orders (payable to:  Town of Plaistow), we DO NOT ACCEPT CASH.
  • For requests that may require photos or videos to be downloaded onto a memory stick and/or a thumb drive, the Requestor will be responsible for the cost of the device. We cannot accept     an external thumb drive or memory stick to download any records due to security concerns.
  • Victims of domestic abuse can obtain a copy of the Incident Report at no cost pursuant to NH RSA 173-B:10; Protection by Peace Officers.
  • Attorneys representing individuals involved in civil litigation may obtain motor vehicle records at the NHDOS, 10 Hazen Drive, Concord, NH by completing a standard DSMV 505 form, pursuant to NH RSA 260:14, V(a)(2).
  • Discovery requests regarding arrests will be directed to the Prosecution Department (603) 378-5434.
