ACCIDENT or INCIDENT REPORT Request
As you enter the main lobby of the Plaistow Police Department, the Records Office window is located to your right. This is where you may request reports or pay fines/fees.
All requests for accident or incident reports, or a log entry should be requested by completing a Request for Reports form (link below). Reports will not be released without authorization from the Chief or an Officer In Charge. There is a processing fee of $1.00 per page, which must be paid on the day you collect the report. We accept credit cards ($2.50 service fee added to payment), or a check or money order can be made payable to the “Town of Plaistow”.
For any requests that may require photographs or videos to be downloaded onto a memory stick and/or thumb drive, the requestor will be responsible for the cost of this device. Due to security concerns, the department cannot accept an external thumb drive or memory stick to download any records.
Accident reports are only available to parties directly involved in the accident such as the owner of a vehicle involved, operator or passenger of a vehicle involved, a pedestrian hit by a vehicle involved, or an owner of any property damaged as a result of an accident. Attorneys representing individuals involved in civil litigation may obtain motor vehicle records at the NH Department of Safety, 10 Hazen Drive, Concord, NH, by completing a standard DSMV 505 form, pursuant to NH RSA 260:14, V(a)(2).
Victims of domestic abuse can obtain a copy of the police report at no cost pursuant to NH RSA 173-B:10; Protection by Peace Officers.
Requests for discovery regarding an arrest will be directed to contact the Prosecution Department at 603-378-5434.
Please note that we do not email or fax reports and the Records Department cannot discuss and/or provide any information contained in the reports. The person requesting the report must be the one collecting the report, in person, and must show an ID at the Records Office within the Police Department.
Please allow a 10-DAY minimum from date of incident for processing of the report.
